Humboldt Independent Practice Association Data Breach Investigation

The Humboldt IPA data breach may have involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

Humboldt IPA is a healthcare organization based in California. Founded in 1995, Humboldt IPA provides administrative and medical management services for a variety of health plans, including Anthem Blue Cross HMO, Blue Lake Rancheria Medical and Dental Health Plans, CalPERS Blue Shield HMO, and IPA Employee Assistance Program.2,3,4 Today, Humboldt IPA works with a network of health care providers dedicated to improving the health of its community.2,3 Headquartered in Eureka, California, Humboldt IPA employs over 10 individuals.

WHAT HAPPENED?

Recently, Humboldt IPA announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on June 28, 2024, Humboldt IPA became aware of a phishing campaign, designed to appear as a legitimate communication from one of its partners.1 As a result, Humboldt IPA launched an investigation to determine the nature of the incident.

Through its investigation, Humboldt IPA confirmed that sensitive personal information in one employee email account may have been viewed and taken by an unauthorized third party between June 26, 2024, and July 1, 2024. As a result, Humboldt IPA began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. Humboldt IPA recently completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Contact information (e.g., mailing address, emergency contact information, email address, telephone number)
  • Date of birth
  • Driver’s license number
  • Medical diagnosis or condition
  • Health insurance information

 

As a result of the breach, Humboldt IPA posted a notice of the incident on its website. Based on the website breach notice, Humboldt IPA is providing affected individuals with a list of the specific types of sensitive information impacted and 12 months of complementary credit monitoring services.
