Strauss Borrelli PLLC, a leading data breach law firm, is investigating Premier Health Partners (“Premier Health”) regarding its recent data breach. The Premier Health data breach involved sensitive personal identifiable information and protected health information belonging to over 10,800 patients.

Premier Health is a healthcare system located across Southwest Ohio. Home to the nation’s 31st largest hospital, Premier Health provides a comprehensive range of care choices at its eight emergency departments, eight Urgent Care locations, and more than 130 outpatient locations and affiliated primary care and specialty physician offices.2 Headquartered in Dayton, Ohio, Premier Health also provides home health, mental health, and substance abuse services.2
WHAT HAPPENED?
In July 2025, Premier Health posted a notice on its website of a cyber event. According to the breach notice posted on its website, on July 12, 2023, Premier Health became aware of suspicious activity within its network environment.1 As a result, Premier Health launched an investigation to determine the nature of the incident.
Through its investigation, Premier Health determined that files on certain Premier Health systems were subject to access and acquisition by an unauthorized party at varying times between June 7 and July 12, 2023. 1 As a result, Premier Health began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. According to the breach notice, the sensitive personal information potentially affected includes the following types of information:
- Name
- Social Security number
- Driver’s license/State-issued identification number
- Passport number
- Medical Information (diagnosis, lab results, provider names, dates of treatment)
- Health insurance information
- Financial account information
- Digital signature
- Username/password
- Date of birth
