WHAT HAPPENED?

Recently, Consulting Radiologists discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its system may have been accessed. Through its investigation, Consulting Radiologists determined that an unauthorized third party accessed this sensitive information on February 12, 2024. On June 14, 2024, Consulting Radiologists began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Medical information
  • Health insurance information
  • Patient face sheets
  • Imaging reports
