WHAT HAPPENED?
On November 12, 2023, HNNCSB discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, HNNCSB determined that an unauthorized third-party may have accessed this sensitive information via a ransomware attack that began on approximately September 26, 2023. Recently, HNNCSB began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Driver’s license number
- Date of birth
- Medical information (e.g., diagnosis/conditions, lab results, medications, other treatment information)
- Health insurance information
