WHAT HAPPENED?
Recently, Arisa Health discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Arisa Health determined that an unauthorized third party accessed this sensitive information between March 1 and March 18, 2024. On July 19, 2024, Arisa Health began notifying individuals whose information may have been impacted. The type of personal information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Email address
- Driver’s license number
- Medical information (e.g., medical record number, certification of substance abuse program completion, medical history and diagnosis)
- Health insurance information (e.g., health insurance number or member ID)
