WHAT HAPPENED?
Recently, MCH discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, MCH determined that an unauthorized third party accessed this sensitive information on April 29, 2024. On June 28, 2024, MCH posted a notice of the incident on its website and stated it will mail breach notification letters to impacted individuals once its investigation process has been completed. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Financial account information
- Medical information
- Health insurance information
