WHAT HAPPENED?

On April 8, 2022, Atrium Health at Home discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its system was accessed. Based on its investigation, Atrium Health at Home determined that an unknown actor had access to this information between April 7 and 8, 2022. On June 7, 2022, Atrium Health at Home began notifying individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Health insurance information
  • Medical information (medical record number, dates of service, medical provider and facility)
  • Medical diagnosis and treatment information
  • Driver’s license number
  • Financial account information
