Strauss Borrelli PLLC, a leading data breach law firm, is investigating Northwest Medical Homes, LLC, which does business as Springfield Family Physicians (“NMH”), regarding its recent data breach. The NMH data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

NMH is a healthcare organization based in Oregon. Today, NMH provides a range of healthcare services to patients of all ages, including physical exams, family planning, immunizations, pediatric care, surgical procedures, cancer screenings, diagnostic imaging, behavioral health, and more.2,3 Additionally, Headquartered in Springfield, Oregon, NMH operates two additional locations in Springfield and Eugene, Oregon.4
WHAT HAPPENED?
Recently, NMH announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on or about May 13, 2025, NMH experienced a data security incident.1 As a result, NMH launched an investigation to determine the nature of the incident.
Through its investigation, NMH confirmed that sensitive personal information in its network may have been accessed by an unauthorized third party between March 19 and May 20, 2025. As a result, NMH began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Medical information
- Health insurance information
As a result of the breach, NMH posted notice of the breach on its website and began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, NMH is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
