Jordan Valley Community Health Data Breach Investigation

WHAT HAPPENED?

Recently, Jordan Valley discovered that it had experienced a data breach in which the sensitive protected health information in its systems may have been accessed. On December 22, 2025, Jordan Valley filed official notice of the data breach with the U.S. Department of Health and Human Services’ Office for Civil Rights.1 Though much information is still not known about the Jordan Valley breach, the U.S. Department of Health and Human Services’ reporting guidelines require entities to report data breaches when they involve protected health information. Therefore, it is likely that the Jordan Valley breach included the protected health information belonging to over 8,800 individuals. As a result of the data breach, Jordan Valley may soon begin notifying individuals whose information may have been impacted.
