WHAT HAPPENED?
Recently, Columbia Medical Practice announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on November 5, 2025, Columbia Medical Practice discovered that an unknown cyber actor accessed a portion of its computer network and installed a computer virus to lock files on computer systems.1 As a result, Columbia Medical Practice launched an investigation to determine the nature of the incident.
Through its ongoing investigation, Columbia Medical Practice has confirmed that sensitive personal information in its systems may have been accessed and acquired by an unauthorized third party during the breach. As a result, Columbia Medical Practice began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Phone number
- Date of birth
- Driver’s license number
- Passport number
- Other government identifier(s)
- Health information (location of health services, dates of service, treatment or condition information, diagnosis, diagnosis code, prescription information, history information, assigned physician
- Financial account number without a security code, access code, or password to access an account
- Health insurance information subscriber or identification number
- Patient account number
As a result of the breach, Columbia Medical Practice posted notice of the breach on its website.
