Indian Health Center of Santa Clara Valley Breach Investigation

Indian Health Center is non-profit health center based in California. Founded in 1977, Indian Health Center provides a range of healthcare services to patients, including medical treatment, dental, counseling and behavioral health, preventative care, vaccination services, inter-tribal services, and more.2,3 Headquartered in San Jose, California, Indian Health Center employs over 10 individuals and offers services across six locations.3,4

WHAT HAPPENED?

Recently, Indian Health Center reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on October 2, 2025, TriZetto Provider Solutions (“TPS”), a third-party administrator that works with Indian Health Center’s electronic medical records provider OCHIN, Inc. (“OCHIN”), became aware of suspicious activity within its web portal that health care provider customers use to access TriZetto systems.1 As a result, TPS launched an investigation to determine the nature of the incident.

Through its investigation, TPS confirmed to Indian Health Center that sensitive personal information in TPS systems related to Indian Health Center may have been accessed by an unauthorized third party from November 2024 to October 2025. As a result, Indian Health Center began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Health insurance member number (may be a Medicare beneficiary identifier)
  • Provider name
  • Health insurer name
  • Primary insured information
  • Other demographic, health, and health insurance information

 

As a result of the breach, Indian Health Center began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, Indian Health Center and TPS are providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
