Strauss Borrelli PLLC, a leading data breach law firm, is investigating Asian and Pacific Islander Wellness Center, Inc., which does business as San Francisco Community Health Center (“SFCHC”) regarding its recent data breach. The SFCHC data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

SFCHC is a community health center based in California. Founded in 1987, SFCHC provides a range of health and wellness services to community members, including healthcare services, homeless services, LGBTQ health services, street medicine services, mental health services, and more.2,3 Headquartered in San Francisco, California, SFCHC employs over 50 individuals and operates six locations across San Francisco.2,3
WHAT HAPPENED?
Recently, SFCHC reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on December 12, 2025, SFCHC was notified by OCHIN, its business associate that supports SFCHC’s electronic health record system, of a data security incident involving one of OCHIN’s subcontractors, TriZetto Provider Solutions (TriZetto), a healthcare eligibility and claims clearinghouse.1 As a result, TriZetto launched an investigation to determine the nature of the incident.
Through its investigation, TriZetto confirmed to SFCHC that sensitive personal information in TriZetto systems related to SFCHC patients may have been accessed by an unauthorized third party between November 2024 and October 2, 2025. As a result, SFCHC began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Health insurance information (member number, health insurer name, provider name, primary insured and dependents)
As a result of the breach, SFCHC began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to California residents, SFCHC and TriZetto are providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
