WHAT HAPPENED?
Recently, Excellent Home Care announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the press release, on November 25, 2025, Excellent Home Care discovered that an employee’s email account was accessed by an unauthorized third party for a brief period.1 As a result, Excellent Home Care launched an investigation to determine the nature of the incident.
Through its investigation, Excellent Home Care confirmed that sensitive personal information in the emails may have been accessed by an unauthorized third party during the breach. As a result, Excellent Home Care began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Phone number
- Date of birth
- Gender
- Medicare/Medicaid number
- Medical information (diagnoses, medications, plan of care documentation)
On December 18, 2025, Excellent Home Care posted notice of the breach in a press release. Additionally, on December 17, 2025, Excellent Home Care began mailing data breach notification letters to impacted individuals. A link to the press release is below.
