Heartland Health Center Inc. Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Heartland Health Center (“Heartland Health”) regarding its recent data breach. The Heartland Health data breach involved sensitive personal information and protected health information belonging to an undetermined number of individuals.

ABOUT HEARTLAND HEALTH CENTER:

Heartland Health is healthcare company based in Nebraska. Today, Heartland Health offers various services such as medical care, dental care and behavioral health care.2 Headquartered in Grand Island, Nebraska, Heartland Health employs over 50 individuals and has additional locations in Ravenna and Grand Island, Nebraska.

WHAT HAPPENED?

Recently, Heartland Health reported to the Attorney General of Massachusetts that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice, on February 4, 2025, Heartland Health discovered it was the victim of an unknown third-party gaining access to its network environment.1 As a result, Heartland Health launched an investigation to determine the nature of the incident.

Through its investigation, Heartland Health confirmed that sensitive personal information in its systems may have been accessed by an unauthorized third party during the breach. As a result, Heartland Health began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Financial account number
  • Username and access information for a non-financial account
  • Medical information (date of medical service, medical diagnosis information, physician or medical facility information, medical condition or treatment information, medical record number, and patient account number)
  • Insurance information (individual health insurance policy number and Medicare or Medicaid number)
  • Certificate or license number
  • Full face photo and referral

 

On October 17, 2025, Heartland Health began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Massachusetts residents, Heartland Health is providing affected individuals with a list of the specific types of sensitive information impacted and 12 months of complimentary credit monitoring services.
