Waveny Lifecare Network, Inc Data Breach Investigation

Recently, Waveny announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice on or around May 28, 2025, Waveny experienced a disruption to its network systems. ¹ As a result, Waveny launched an investigation to determine the nature of the incident.

Through its investigation, Waveny confirmed that sensitive personal information in its systems may have been accessed and acquired by an unauthorized third party during the breach. As a result, Waveny began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

• Name
• Social Security number
• Address
• Date of birth
• Admission date
• Discharge date
• Date of death
• Phone number
• Email address
• Medical information (medical record number, medical imaging results, patient account number, facial photographic images, laboratory test results, medical imaging results, and electronic health records)
• Health insurance information (health insurance account or policy number, payment information, and Medicare or Medicaid information)
• Financial account number
