Family & Community Services Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Family & Community Services, Inc. (“FCS”) regarding its recent data breach. The FCS data breach may have involved sensitive personal identifiable information belonging to an undetermined number of individuals.

ABOUT FAMILY & COMMUNITY SERVICES, INC.:

FCS is a nonprofit social services agency based in Ohio. Founded in 1944, FCS provides families and individuals of all ages with social assistance through food, clothing, shelter, counseling, and mentoring services.3 Additionally, FCS offers these services through more than 71 programs across 21 counties in Ohio and Michigan. 3 Headquartered Ravenna, Ohio, FCS employs over 500 individuals and approximately 3,500 volunteers.3

On January 1, 2024, FCS completed a merger with the company AxessPointe Community Health Centers (“AxessPointe”), a federally qualified health center based in Ohio that provides primary medical, dental, preventative health, behavioral health, and OB-GYN services in medically underserved areas.4,5 Through this merger, FCS and AxessPointe formed the new entity Axess Family Services, which aims to enhance access to comprehensive healthcare, behavioral healthcare, and social services across its service area. 2,6

WHAT HAPPENED?

Recently, FCS announced that it had experienced a data breach in which sensitive personal identifiable information in its care may have been compromised. According to the notice shared on its website, on May 22, 2025, FCS discovered that its systems were potentially compromised by an unauthorized actor.1 As a result, FCS launched an investigation to determine the nature of the incident.

As of September 19, 2025, the FCS investigation is ongoing and any information that may have been accessed has not yet been determined. Based on the website notice, FCS will mail any affected individuals and offer complimentary resources such as credit monitoring and identity protection services. 1 A link to the website breach notice is below.
