Integrity is a drug testing organization for Department of Transportation (DOT) regulated industries based in Michigan. Partnered with Frontier Airlines, a Denver-based airline company, Integrity provides federally-mandated drug and alcohol testing in accordance with rules issued under the DOT, as well as medical review officer services, policy and procedural development, and nationwide collection site management.3,5 Headquartered in Sterling Heights, Michigan, Integrity employs over 10 individuals and services aviation, motor carriers, mass transit and other transportation services regulated by the DOT.4

WHAT HAPPENED?

Recently, Integrity reported to the Attorney General of New Hampshire that it had experienced a data breach in which sensitive personal identifiable information in its care may have been compromised. According to the breach notice, on or about March 27, 2025, Integrity experienced a data security incident in which there was unauthorized access to its networks.1 As a result, Integrity launched an investigation to determine the nature of the incident.

Through its investigation, Integrity confirmed that the sensitive personal information in its systems may have been accessed or acquired by an unauthorized third party during the breach.1 As a result, Integrity began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Date of birth

 

On August 15, 2025, Integrity began mailing data breach notification letters to impacted individuals. Based on the breach notice being sent to New Hampshire residents, Integrity is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services. 1 A link to the letters that Integrity filed with the Attorney General of New Hampshire is below.
