UI Community HomeCare is a University of Iowa Health Care affiliate company that provides full-service home infusion and medical equipment services to individuals living in Iowa, western Illinois, and northern Missouri.1 Headquartered in Iowa City, Iowa, UI Community HomeCare operates out of three locations.
WHAT HAPPENED?
Recently, UI Community HomeCare announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been compromised. According to the breach notice, on July 3, 2025, an unauthorized individual accessed UI Community HomeCare’s computer system without permission.1 As a result, UI Community HomeCare launched an investigation to determine the nature of the incident.
Through its investigation, UI Community HomeCare confirmed that sensitive personal information belonging to customers and UI Health Care patients in its systems may have been viewed and copied by an unauthorized third party during the breach. As a result, UI Community HomeCare began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Phone number
- Medical information (e.g., medical record number, provider, dates of service, type of visit)
- Health insurance information
As a result of the breach, UI Community Care posted a notice of the incident on its website. Additionally, on August 29, 2025, UI Community HomeCare began mailing data breach notification letters to impacted individuals. Based on the website breach notice, UI Community HomeCare is providing affected individuals with a list of the specific types of sensitive information impacted.
