Fundamental Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Fundamental Administrative Services, LLC (“Fundamental”), regarding its recent data breach. The Fundamental data breach may have involved sensitive personal identifiable information and protected health information belonging to over 56,000 patients.

Fundamental is a healthcare company based in Maryland that offers advanced nursing, medical, and rehabilitation services. Through its network of covered entities, Fundamental offers services related to nursing, subacute care, long-term basic medical care, outpatient rehabilitation, Alzheimer’s & related disorders, specialty hospital services, and assisted living.2 Headquartered in Sparks Glencoe, Maryland, Fundamental has additional facilities in Indiana, Maryland, Nevada, New Mexico, South Carolina, Texas, and Wisconsin.3

WHAT HAPPENED?

Recently, Fundamental discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed.1 According to the breach notice, Fundamental became aware of suspicious activity on its computer network.As a result, Fundamental launched an investigation to determine the nature of the incident.

Through its investigation, Fundamental determined that between October 27, 2024, and January 13, 2025, an unauthorized actor copied certain files and folders from the computer network used by Fundamental’s covered entities.1 As a result, Fundamental began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license number or state identification number
  • Date of birth
  • Financial account information
  • Medical treatment information
  • Health insurance information
  • Medicare/Medicaid plan name

 

As a result, Fundamental posted notice of the incident on its website. Based on the breach notice, Fundamental is providing affected individuals with a list of the specific types of sensitive information impacted.
