Founded in 1981, Eleos Wellness offers a full spectrum of services, from crisis intervention and emergency screenings to inpatient care and community programs.2 Headquartered in Pinellas Park, Florida, Eleos Wellness has an additional location in Largo, Florida.2
Eleos Wellness is a part of Solutions for Administrative Services, Inc. (SAS), which is an approved provider of continuing education for individuals or organizations that conduct training accepted by the Florida Certification Board (FCB) for renewal purposes, and which reflects the current needs of certified professionals.3 Solutions for Administrative Services, Inc. (SAS) currently does business under the business name Operation PAR.4
WHAT HAPPENED?
Recently, Eleos Wellness posted a notice on its website regarding a cyber event. According to the breach notice posted on its website, on June 11, 2025, Solutions for Administrative Services, Inc. (SAS) learned certain systems within Eleos Wellness’s network environment were affected by a data security incident.1 As a result, Eleos Wellness launched an investigation to determine the nature of the incident.
At this time, Eleos Wellness’s investigation into the incident and what specific information was impacted is ongoing. 1 While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Health insurance information
