Strauss Borrelli PLLC, a leading data breach law firm, is investigating Operation PAR, Inc. (“PAR”) regarding its recent data breach. The PAR data breach involved sensitive personal identifiable information and protected health information belonging to an undetermined number of people.

PAR is a mental health service and substance use disorder treatment clinic located in Florida. Founded in 1970, PAR provides a range of services such as residential treatment, prevention methods, outpatient services, medication-assisted treatment, and medical detoxification.2,3 Headquartered in Pinellas Park, Florida, PAR has additional locations in St. Petersburg, Sarasota, Port Charlotte, Port Richey, Spring Hill, Fort Myers, Clearwater, Inverness, Bradenton, and Largo, Florida.4

WHAT HAPPENED?

Recently, PAR posted a notice on its website of a cyber event. According to the breach notice posted on its website, on June 10, 2025, PAR learned certain systems within its network environment were affected by a data security incident.As a result, PAR launched an investigation to determine the nature of the incident.

At this time, PAR’s investigation into the incident and what specific information was impacted is ongoing. 1 While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Health Insurance information
