San Francisco Campus for Jewish Living Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Hebrew Home for Aged Disabled, which does business as San Francisco Campus For Jewish Living (“SFCJL”), regarding its recent data breach. The SFCJL data breach involved sensitive personal information and protected health information belonging to over 2,500 individuals.

SFCJL is a senior services organization based in California. Founded in 1871, SFCJL is located on a on a nine-acre campus in San Francisco’s Excelsior neighborhood, and includes the Frank Residences, offering premium assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.3 Today, SFCJL serves over  2,500 individuals annually.4 Headquartered in San Francisco, California, SFCJL employs over 500 individuals.

WHAT HAPPENED?

Recently, SFCJL announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been accessed. According to the breach notice, on December 27, 2024, SFCJL discovered that an unauthorized user gained access to an employee’s email user account.1 As a result, SFCJL launched an investigation to determine the nature of the incident.

Through its investigation, SFCJL confirmed that sensitive personal information in its systems may have been compromised by an unauthorized third party during the breach. As a result, SFCJL began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On April 2, 2025, SFCJL completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Medical information (e.g., medical record number, dates of service, admission/discharge information, medication information, COVID testing information, payment history)
  • Insurance information

 

On April 7, 2025, SFCJL posted a notice of the incident on its website. Additionally, on the same day, SFCJL began sending breach notification letters to impacted individuals. Based on the website breach notice, SFCJL is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
