CDC is a nonprofit organization and dental healthcare provider based in Minnesota. Founded in 2004, CDC’s preventive dentistry includes exams, cleanings, radiographs, sealants, and oral hygiene instruction.3 Additionally, CDC offers restorative services such as fillings, crowns, bridges, root canals, dentures, and partials.3 Today, CDC provides dental care for 110,000 patient visits each year through five clinics in Minnesota and outreach in community settings.4 Headquartered in Maplewood, Minnesota, CDC employs over 275 individuals.
WHAT HAPPENED?
Recently, CDC announced that the sensitive personal identifiable information and protected health information in its care may have been accessed and acquired. According to the breach notice, on December 20, 2024, CDC found activity in its computer system that happened without its permission.1 As a result, CDC launched an investigation to determine the nature of the incident.
Through its investigation, CDC confirmed that sensitive personal information in its system may have been viewed and obtained by an unauthorized third party on or around December 6, 2024. As a result, CDC began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On March 24, 2025, CDC completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Driver’s license number or other government issued identification number
- Passport number
- Medical information
- Health insurance information
On March 28, 2025, CDC posted a notice of the incident on its website. As the investigation is ongoing, CDC will update its notification if additional types of impacted information are identified.
