Palmetto Subacute Care Center Data Breach Investigation

Strauss Borrelli PLLC, a leading data breach law firm, is investigating Palmetto Operating, LLC, which does business as Palmetto Subacute Care Center (“Palmetto”), regarding its recent data breach. The Palmetto data breach involved sensitive personal information and protected health information belonging to over 2,700 individuals.

ABOUT PALMETTO OPERATING, LLC D/B/A PALMETTO SUBACUTE CARE CENTER:

Palmetto is a healthcare facility in Florida. Today, Palmetto offers a variety of services to individuals, including daily physical, occupational, and speech therapy, subacute post-operative care, outpatient therapy, 24/7 skilled nursing, complex medical care, respiratory care, IV therapy, wound care, diabetic management, and more.3  Additionally, Palmetto provides various specialty programs regarding orthopedic and short term rehabilitation, wound care, long term care, amputee care, and recreation. Located in Miami, Florida, Palmetto is a part of the CareRite Centers network which spans New York, New Jersey, Tennessee, and Florida.4

WHAT HAPPENED?

Recently, Palmetto released a notice to the press stating that the sensitive personal identifiable information and protected health information in its care may have been compromised. According to the press release, on or about September 19, 2024, Palmetto learned of a data security incident involving a third party vender.1 As a result, Palmetto launched an investigation to determine the nature of the incident.

Through its investigation, Palmetto confirmed that sensitive personal information of current or past patients may have been compromised by an unauthorized third party on or around July 20, 2024. As a result, Palmetto began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Medical information

 

On January 28, 2025, Palmetto began mailing data breach notification letters to impacted individuals. Based on the press release, Palmetto is providing affected individuals with a list of the information impacted. A link to Palmetto’s press release is below.
