Community Care Alliance Data Breach Investigation

Community Care Alliance is a non-profit organization based in Rhode Island. Founded in 1891, Community Care Alliance has 50 programs focused on helping people challenged by mental health and addictions, basic needs, trauma, housing, education and employment.2 Today, Community Care Alliance accepts walk-ins and appointments, and accepts Managed Medicaid, Commercial United, Medicare, BC/BS, and Tri-Care.3 Headquartered in Woonsocket, Rhode Island, Community Care Alliance has 10 locations in Rhode Island.4

WHAT HAPPENED?

Recently, Community Care Alliance announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, Community Care Alliance recently discovered unusual activity on its network.1 As a result, Community Care Alliance launched an investigation to determine the nature of the incident.

Through its investigation, Community Care Alliance confirmed that sensitive personal information in its systems may have been viewed and obtained by an unauthorized third party between July 1 and July 5, 2024. As a result, Community Care Alliance began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On or around January 8, 2025, Community Care Alliance completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Driver’s license number
  • Medical information, such as diagnosis/condition, lab results, medications, patient ID number, provider name, and treatment information
  • Health insurance information

 

As a result of the data breach, Community Care Alliance posted a notice of the incident on its website. Based on the website breach notice, Community Care Alliance is mailing affected individuals and providing a list of the specific types of sensitive information impacted.
