WHAT HAPPENED?

Recently, Jaime S. Schwartz MD, PC reported to the Attorney General of California that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on June 27, 2024, Jaime S. Schwartz MD, PC discovered that an unauthorized third party utilized a third-party vendor’s credentials to access the practice’s medical billing and practice management system.1 As a result, Jaime S. Schwartz MD, PC launched an investigation to determine the nature of the incident.

Through its investigation, Jaime S. Schwartz MD, PC confirmed that sensitive personal information in its systems belonging to patients may have been viewed and obtained by an unauthorized third party during the breach. As a result, Jaime S. Schwartz MD, PC began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:

  • Name
  • Patient images
  • Address
  • Date of birth
  • Medical information
  • Prescription medications
  • Health insurance information

 

In January 2025, Jaime S. Schwartz MD, PC began mailing data breach notification letters to impacted individuals. Based on the breach notices provided to California residents, Jaime S. Schwartz MD, PC is providing affected individuals with a list of the information impacted and 12 months of complimentary credit monitoring services.
