FDHS is a digestive health network based in Florida. Founded in 2011, FDHS specializes in screening, diagnosing, and treating all gastroenterology disorders and conditions.3 Some of the disorders treated by FDHS include ulcerative colitis, Crohn’s disease, constipation, diarrhea, rectal bleeding, abdominal pain, and hemorrhoids.3 Headquartered in Bradenton, Florida, FDHS has over 20 locations throughout Florida and employs over 200 individuals.
WHAT HAPPENED?
Recently, FDHS reported to the Attorney General of the Commonwealth of Massachusetts that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. According to the breach notice, on March 28, 2024, FDHS discovered a data security incident in which an unauthorized third party accessed its computer network. As a result, FDHS launched an investigation to determine the nature of the incident.
Through its investigation, FDHS learned that sensitive personal information may have been viewed and taken by an unauthorized third party. As a result, FDHS began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. On June 14, 2024, FDHS completed this review. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license number
- Medical records
On August 13, 2024, FDHS began mailing data breach notification letters to impacted individuals. Based on the breach notice sent to Massachusetts residents, FDHS is providing affected individuals with a list of the specific type of sensitive information impacted and 12 months of complimentary credit monitoring services.
