WHAT HAPPENED?

On October 23, 2023, Burr & Forman discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Burr & Forman determined that an unauthorized third party accessed this sensitive information on September 30, 2023. On January 9, 2024, Burr & Forman began notifying individuals whose information may have been impacted. The type of personal information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Medical coding information with dates and descriptions 
  • Insurance information
