WHAT HAPPENED?

Recently, Lakeview Health discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Lakeview Health determined that an unauthorized actor gained access to this sensitive information on or around January 25, 2024. On May 29, 2024, Lakeview Health began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Address
  • Driver’s license number
  • Financial account number
  • Medical information (e.g., patient ID, medical treatment information, diagnosis or conditions, prescription information) 
  • Health insurance information
