WHAT HAPPENED?
In June 2022, Interim discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed or acquired. Through its investigation, Interim determined that an unauthorized actor may have accessed this sensitive information between April 29, 2022, and July 3, 2022. On April 25, 2024, Interim began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Address
- Driver’s license number
- Medical information (e.g., diagnosis, lab results, medications, other treatment information)
- Health insurance information
