Strauss Borrelli PLLC, a leading data breach law firm, is investigating Continuum Health Alliance, LLC, and its subsidiary Consensus Health (“Continuum”) regarding its recent data breach. The Continuum data breach involved sensitive personal identifiable information and protected health information belonging to over 377,000 individuals.

Founded in 2018, Continuum affords its member physicians clinical autonomy, allowing them to truly focus on the delivery of high levels of patient care within the local markets they serve throughout the state.3 With more than 150 independent providers and nearly 50 practices comprising its medical group, Continuum’s presence spans 17 of the state’s 20 counties.3 The advantages of being part of the Continuum model include, among others, access to various administrative services, technology platforms and value-based programs that allow member physicians to focus primarily on the patient and elevate the care they provide.3 Headquartered in Marlton, New Jersey, Continuum employs over 500 individuals.
WHAT HAPPENED?
Recently, Continuum discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Continuum determined that an unauthorized actor may have accessed this sensitive information between October 18 and October 19, 2023. On February 16, 2024, Continuum posted a notice of the breach on its website and on April 29, 2024, Continuum began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Medical information (e.g., treatment and/or diagnosis code, provider name, dates of treatment)
- Health insurance information
