WHAT HAPPENED?
On March 4, 2024, PCMH discovered that it had experienced a data breach in which personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, PCMH determined that an unauthorized party may have accessed this sensitive information beginning on March 7, 2023. On March 25, 2024, PCMH began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Phone number
- Email address
- Driver’s license and/or government ID number
- Vehicle identification number
- Passport number
- Financial information (e.g., account number, credit and/or debit card number)
- Medical record number
- Patient ID account number
- Medical and/or health information
