WHAT HAPPENED?

Recently, American Vision Partners discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information may have been accessed and acquired. Through its investigation, American Vision Partners determined that an unauthorized party may have accessed this sensitive information on November 14, 2023. On February 6, 2024, American Vision Partners began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Date of birth
  • Contact information
  • Medical information (e.g., services received, clinical records, medications)
  • Insurance information
