WHAT HAPPENED?
On August 2, 2023, Sirius Federal discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Sirius Federal determined that an unauthorized actor may have accessed and acquired this sensitive information between July 31, 2023, and August 2, 2023. On January 19, 2024, Sirius Federal began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

- Social Security number
- Address
- Date of birth
- Government issued ID number
- Financial information (e.g., account information, credit/debit card information)
- Medical information
- Health insurance information
- Taxpayer identification number
