WHAT HAPPENED?
On November 14, 2023, COA discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, COA determined that an unauthorized party may have accessed this sensitive information as early as October 4, 2023. Recently, COA began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Contact information (e.g., phone number, email address)
- Medical information (e.g., dates of service, nature or type of services, diagnoses, conditions, prescriptions, test results, medications, other treatment information)
- Insurance type and ID
