WHAT HAPPENED?

On or about December 21, 2022, MedInform discovered that it had experienced a data breach in which the sensitive personal identifiable information in its systems belonging to Cleveland Clinic patients may have been accessed and acquired. Through its investigation, MedInform determined that an unauthorized actor may have accessed and obtained this sensitive personal identifiable information between December 5 and December 21, 2022. On May 24, 2023, MedInform began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Financial account information
  • Medical billing information
  • Address
