Dental Health Management Solutions Data Breach Investigation

DHMS is a dental health provider that specializes in offering comprehensive dental services to government and public sector agencies. Founded in 2003, DHMS set out to provide dental services to assisted living centers, quickly transitioning to offer similar services to military members, government agencies, correctional facilities, and more. Accordingly, DHMS focuses on mobile and portable dentistry services, utilizing three mobile dental vehicles to serve patients unable to seek treatment in a traditional clinic setting. Additionally, DHMS provides medical staffing services and logistic management. DHMS is headquartered in Cedar Park, Texas.

WHAT HAPPENED?

On or about August 20, 2021, DHMS discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, DHMS determined that an unauthorized actor may have accessed this sensitive information through an employee email account on August 5, 2021. On February 17, 2023, DHMS began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Driver’s license number
  • Address
  • Financial information (e.g., account number, routing number)
  • Health insurance information (e.g., Medicaid identification number)
  • Medical information
