WHAT HAPPENED?
August 30, 2022, Crown Uniform & Linen Service discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Crown Uniform & Linen Service determined that an unauthorized actor may have accessed this sensitive information between August 29 and September 6, 2022. On November 30, 2022, Crown Uniform & Linen Service began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Driver’s license number
- Financial information (e.g., direct deposit information, 401k information)
- Medical information (e.g., workers’ compensation claims, drug testing information, physical exam information)
