WHAT HAPPENED?
Recently, FSH discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, FSH determined that an unauthorized actor may have accessed and acquired this sensitive information on November 4, 2023. On December 21, 2023, FSH began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Address
- Date of birth
- Medical and/or treatment information (e.g., provider and facility names, medical record number or other patient identifier, diagnosis information, procedure information)
