WHAT HAPPENED?
On December 4, 2023, Meridian discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Meridian determined that an unauthorized actor may have accessed this sensitive information on August 11, 2023. On December 22, 2023, Meridian began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Medical information (e.g., medical diagnosis and treatment information, prescription information)
- Health insurance information
