WHAT HAPPENED?
Recently, R1 discovered that it had experienced a data breach in which personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, R1 determined that an unauthorized party may have accessed sensitive information belonging to patients of St. Rose Dominican Hospital de Lima, a Dignity Health hospital, on November 17, 2023. As a result, R1 began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Contact information
- Medical information (e.g., location of services, clinical and/or diagnosis information, patient account and/or medical record number)
