ABOUT HMG HEALTHCARE, LLC:

HMG is a nursing home and residential care company based in Texas. Offering 24-hour care and assistance, HMG provides a variety of services, including skilled nursing, personalized rehabilitation, memory care, assisted living, independent living, and advanced pulmonary care. With over 30 locations across Texas and Kansas, HMG provides care for more than 3,500 residents and patients. Headquartered in The Woodlands, Texas, HMG employs over 5,000 individuals, including more than 4,100 dedicated caregivers.

WHAT HAPPENED?

In November 2023, HMG discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, HMG determined that an unauthorized actor may have accessed and acquired this sensitive information in August 2023. In January 2024, HMG began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Contact information
  • Driver’s license or government issued ID number
  • Financial information (e.g., account number, credit or debit card number)
  • Medical information (e.g., general health information, medical treatment information)
  • Health insurance information
  • Employment records
