WHAT HAPPENED?
On May 12, 2023, Aria discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Aria determined that an unauthorized actor may have accessed personal and protected health information belonging to certain current and former patients of its affiliated vision provider companies. On January 19, 2024, Aria began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license number
- Date of birth
- Medical information (e.g., medical treatment and/or diagnosis information)
- Health insurance information
