Strauss Borrelli PLLC, a leading data breach law firm, is investigating Colorado Ophthalmology Associates (“COA”) regarding its recent data breach. The COA data breach involved sensitive personal identifiable information and protected health information belonging to over 6,000 individuals.

Founded in 1960, COA provides comprehensive vision care to families, including routine eye exams, state-of-the art cataract surgery, medical and surgical glaucoma care, LASIK, and refractive surgery to correct distance and near vision, astigmatism, and corneal disease.2 Additionally, COA offers full service optical shops and contact lens departments.2 Headquartered in Denver, Colorado, COA has an additional location in Lakewood, Colorado, and employs over 6 doctors.

WHAT HAPPENED?

On November 14, 2023, COA discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, COA determined that an unauthorized party may have accessed this sensitive information as early as October 4, 2023. Recently, COA began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Contact information (e.g., phone number, email address)
  • Medical information (e.g., dates of service, nature or type of services, diagnoses, conditions, prescriptions, test results, medications, other treatment information)
  • Insurance type and ID
