WHAT HAPPENED?
Recently, City of Hope discovered that it had experienced a data breach in which personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, City of Hope determined that an unauthorized party may have accessed this sensitive information between September 19, 2023, and October 12, 2023. On April 2, 2024, City of Hope began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license or other governmental identification
- Financial details (e.g., bank account number and/or credit card details)
- Health insurance information
- Medical record number
- Medical information
