WHAT HAPPENED?

Recently, Pacific Guardian Life discovered that it had experienced a data breach in which personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Pacific Guardian Life determined that an unauthorized actor may have accessed this sensitive information through its email environment on August 25, 2023. On April 4, 2024, Pacific Guardian Life began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Driver’s license or state ID number
  • Date of birth
  • Financial account information
  • Payment card information
  • Medical information
  • Health insurance information
  • Taxpayer ID number
  • Copy of marriage certificate
  • Mother’s maiden name
  • Electronic signature
