WHAT HAPPENED?
Recently, SRBHH discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, SRBHH determined that an unauthorized actor may have accessed this information between January 27 and January 28, 2024. On April 26, 2024, SRBHH began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Driver’s license number
- Date of birth
- Medical information (e.g., medical record number, services received, dates of service, treating physician)
