EHG is a nursing home care provider based in Washington. Founded in 2022, EHG provides healthcare services to patients and residents across its range of care communities, including nursing communities, assisted living communities, independent living communities, and memory care centers.2,3,4 Services offered at communities managed by EHG include nursing care, rehabilitation therapy, respite stays, memory support, and more.3 Headquartered in Vancouver, Washington, EHG employs over 1,000 individuals and provides care at over 60 locations across California, Washington, Oregon, Nevada, South Dakota, Wyoming, and Montana.3,4
WHAT HAPPENED?
Recently, EHG announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on or about December 3, 2025, EHG became aware of unauthorized activity within its cloud-based healthcare platform.1 As a result, EHG launched an investigation to determine the nature of the incident.
Through its investigation, EHG confirmed that sensitive personal information in its care may have been accessed by an unauthorized third party during the breach. As a result, EHG began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Medical information
On February 25, 2026, EHG posted notice of the breach on its website. Based on the website breach notice, EHG is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
