Apex is a neurosurgery and spinal care practice based in Georgia. Founded in 2020, Apex provides a range of spinal and neurological services for patients, including spine surgery, spinal decompression, herniated discs, back pain, neck pain, and more.2,3 Headquartered in Suwanee, Georgia, Apex employs over 10 individuals and operates two additional locations in Columbus, Georgia and Bethlehem, Georgia.2,3
WHAT HAPPENED?
Recently, Apex announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on December 23, 2025, Apex learned that an unknown cyber actor accessed a portion of its computer network and installed a computer virus to lock files on computer systems.1 As a result, Apex launched an investigation to determine the nature of the incident.
Through its investigation, Apex confirmed that sensitive personal information in its network may have been accessed and acquired by an unauthorized third party on December 9, 2025. As a result, Apex began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Phone number
- Date of birth
- Driver’s license number
- Passport number
- Other government identifiers
- Health information (health services, dates of service, treatment or condition information, diagnosis, diagnosis code, prescription information, history information, assigned physician)
- Financial account number without security code, access code, or password
- Health insurance information subscriber or identification number
- Patient account number
As a result of the breach, Apex posted notice of the breach on its website. Based on the website breach notice, Apex is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
