Strauss Borrelli PLLC, a leading data breach law firm, is investigating Escambia Community Clinics, which does business as Community Health Northwest Florida (“CHNF”), regarding its recent data breach. Founded in 1992, CHNF provides a range healthcare services to patients, including walk-in care services, adult and family care, behavioral health, pediatric care, dental care, pharmacy, and more.2,3 Headquartered in Pensacola, Florida, CHNF employs over 200 individuals and offers services across approximately 18 locations.3,4
WHAT HAPPENED?
Recently, CHNF announced that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its care may have been compromised. According to the breach notice shared on its website, on December 24, 2024, CHNF became aware of suspicious activity within its network environment.1 As a result, CHNF launched an investigation to determine the nature of the incident.
Through its investigation, CHNF confirmed that sensitive personal information in its systems may have been accessed by an unauthorized third party during the breach. As a result, CHNF began a review of the data to determine what information had been impacted as well as identify the specific individuals affected. While the information impacted varies depending on the individual, the type of information potentially exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license or state identification card number
- Financial account number
- Credit or debit card numbers
- Patient identification and medical record number
- Medical information
- Health insurance information
On January 26, 2026, CHNF posted notice of the breach on its website and began mailing data breach notification letters to impacted individuals. Based on the website breach notice, CHNF is providing affected individuals with a list of the specific types of sensitive information impacted and complimentary credit monitoring services.
